AUTOMATED APPOINTMENT REMINDER SERVICE

We have an automated reminder service that will remind you of your appointment 1 week before and 1 day before your appointment. Please text back to confirm your appointment.  

APPOINTMENTS

As a courtesy to our office and our other patients who may be trying to get in, we require at least 24 hours notice of cancellation of your appointment or you may be subject to a missed appointment charge.

For initial appointments scheduled, we do require a Credit Card number to put on file. The card will NOT be charged unless there is a missed appointment or less than 24 hour cancellation.

PAYMENTS

We require payment at time of service. We accept all credit cards and Care Credit. If a payment arrangement has been made, any balance that remains on your account is subject to a finance charge. Payment must be made within the last 30 days. After 90 days, your account will be placed in collections and we will cancel all future appointments.

INSURANCE

We do not accept Medicaid, Wellcare & Passport.

Because there are so many insurance options available and a wide variety of factors can affect your coverage, we strongly recommend that you contact your insurance carrier prior to your visit. A call with your carrier about the amount and extent of your coverage and if we are In Network with your plan will help prevent any confusion, misunderstandings, or frustration about your financial responsibility for certain services. Being In Network will cover more costs on your procedures. Please call your insurance before your scheduled appointment to confirm your benefits.  The patient is responsible for the remaining balance.
